Some chronic diseases require regular followup for blood tests or other examinations and we run a series of clinics for the more common chronic diseases. Other diseases will be followed up in routine surgery appointments
The diabetic clinic offers annual diabetic checks (or more frequently if required). Clinics are run weekly and patients are recalled automatically according to the recall date set on the computer system.
All diabetics are sent a recall letter with a date and time of appointment - two appointments are given: the first is with the practice nurse. At this appointment she will take blood and complete the relevant checks required.
The review appointment will be 2 weeks later with the doctor/practice nurse. You will be sent a Care Planning Booklet to bring with you to this appointment. The results of your blood tests will be discussed to determine your ongoing treatment. These review appointments are held on Monday mornings and Tuesday afternoons.
The appointment letter includes information on how to book the transport service run by local volunteers. For more details contact the administration team on the surgery number.
This is held on a Wednesday by the practice nurse. All asthmatics will be called in for an annual review.
The Chronic Obstructive Pulmonary Disease clinic runs by appointment on a Wednesday. It is run by the practice nurse and patients are called in for an annual review.
These are held on Monday afternoons, Tuesday and Thursday mornings, by appointment with the Practice Nurse. Patients will be offered appointments yearly for review or on demand as required.
